What usually belongs here in Germany

For orientation, not legal advice: the provider’s name and a postal address, an e-mail address for quick contact, for editorial content the person responsible; for businesses additionally the legal form, representatives, register and VAT ID. For a private, free website, name, address and e-mail are usually enough. Whether an imprint is required depends on the individual case.
